The Tamil Nadu government provides home-based medical care primarily through its flagship Makkalai Thedi Maruthuvam (MTM) scheme, which delivers services directly to households. This includes home visits by health teams for noncommunicable disease management, essential drug delivery, and follow-ups for conditions like diabetes and hypertension. The government also offers a three-month certificate course for home-based elderly care support assistants through its medical colleges, addressing the growing need for elder care. 
Makkalai Thedi Maruthuvam (MTM) Scheme 
Objective: To bring holistic and comprehensive healthcare services to people's doorsteps, ensuring a continuum of care and meeting health needs at home.
Services:
Management of noncommunicable diseases (NCDs).
Delivery of essential medicines for conditions like diabetes and hypertension.
Routine investigations and follow-up care.
Team: Field teams include women's health volunteers, physiotherapists, and palliative care nurses. 
Home-Based Elderly Care Support Assistant Program 
Objective: To train individuals to provide care and support to the elderly in their homes.
Program details:
A three-month certificate course is offered in government medical colleges across the state.
Includes practical training on tasks like assisting with daily needs (bathing, feeding), basic health checks, administering medication, and basic life support.
Eligibility: Candidates typically need to have completed their SSLC. 
Other relevant initiatives
Home-Based Care for Young Child Programme (HBYC): Supports families with young children by providing counselling and help with nutrition, growth monitoring, and managing common illnesses.
Government health infrastructure: Urban Primary Health Centres (UPHCs) and Urban Health Sub-Centres (HSCs) act as first points of contact, focusing on preventive healthcare and outreach activities within communities.
